Advance Directive Checklist

Completing your advance care plan, or otherwise known as advance directive care plan, is the first step toward having your end-of-life wishes honored. This could be in the form of a living will, New York State’s Health Care Proxy or power of attorney form. Be sure to follow these steps after you complete your plan.
